Your Right to a Good Faith Estimate
If you don’t have health insurance, or you choose not to use your insurance, you have the right to receive a “Good Faith Estimate” of the expected charges for your care at Flagstaff Psychiatric Care.
Under federal law, we provide this estimate in writing prior to your appointment so you know what to expect before your care begins. It lists the services we expect to provide, the expected billing codes, and the total expected charges.
How to request one
You can ask for a Good Faith Estimate at any time, including before you schedule an appointment. Call us at (928) 393-8013 or email admin@flagstaffpsychiatriccare.com. We will give you a written estimate within the time required by law, and automatically when you schedule a visit.
Your right to dispute a bill
If you are billed for at least $400 more than the total on your Good Faith Estimate, you may be able to dispute the bill. Please keep a copy of your estimate and any bills you receive.
